Instructions for a Telemedicine Appointment with Dr. Dieguez.
Dr. Dieguez is committed to keeping everyone safe during the post COVID-19 pandemic period, specially our elderly patients and others with chronic conditions, who are our most vulnerable. Here are the instructions for you the patient to follow:
1. CONTACT OUR OFFICE. Call our office at (904) 824-0955 during our regular business hours to schedule a Telemedicine appointment at a time that is convenient for you. Our staff will ask you some questions including billing information if we do not already have it. You will need to provide us with the cell phone number and/or e-mail address (your choice) of your video capable “smart devise” where you can receive text and e-mails and have video capability. This can be a cell phone, a tablet or a computer with Internet connection. The preferred browser is Google Chrome. Internet Explorer is not supported.
2. BE PREPARED. Please be ready five minutes prior to the scheduled time agreed with your smart devise. You will receive either a text message or an e-mail from Dr. Dieguez. You will have ten minutes to answer or you will be disconnected and miss your appointment!

3. CLICK ON THE LINK RECEIVED. Click on either of the links received and follow instructions. The first time you use video chat, your devise may ask you for permision to use the camera and microphone. Please allow access. Dr. Dieguez is waiting for you on the other end to connect with you for you Telemedicine visit. He will be able to order most any test or prescriptions he deems necessary for you.
4. FOR NEW PATIENTS. If you are a new patient you can also participate in a Telemedicine appointment. However before you do, you will need to download, complete, sign and return to us the new patient paperwork which you can find under the following link :

You can fax it back to (904) 824-2226 or (904) 512-5255. When we receive it, the doctor reviews it and we will call you to set up your Telemedicine appointment with Dr. Dieguez.
