Why choose an Orthobiologic Medicine/Interventional Orthopedic Clinic like ours and not a traditional “Pain Clinic”?
In this writing we will try to highlight the main differences between the two clinics in a short concise fashion without going into many details.
The differences are many. In the traditional “Pain Clinic” the aim is to control the symptom of pain without regard to actually healing or improving the condition of the tissues causing the pain. It is basically a symptomatic treatment of the pain.
What are the methods or techniques used in each type of clinic mentioned above?
A) The Traditional “Pain Clinic”.
In the traditional “Pain Clinic” this is mainly done with one or more of these five methods:
1.- Injection procedures of high dose steroids.- The problems with this methods are two folds: The effects do not last, and also in the long run they weaken and damages the tissueseven more leading to increased pain and eventually to surgery.
2.- The use of potent and risky opioid pain pills with the potential of creating addiction and all its drawbacks, as we are seeing on the news nowadays on a regular basis.
3.- By using certain tissue destroying procedures such as radiofrequency ablation, cryotherapy, or neurolitic blocks.
4.- By implanting foreign bodies such as spinal cord stimulators or peripheral nerve stimulator with the potential of infection, rejection or damage to bodily structures during the procedure to implant it or when taking it out if needed.

B) The “Orthobiologic Medicine/Interventional Orthopedic Clinic”.
In the “Orthobiologic Medicine/Interventional Orthopedic Clinic”, the methods used are more advanced and more natural.
They are aimed at treating pain-producing conditions by healing or improving the damaged tissues and not by weakening tissues or destroying tissues and without all the drawbacks of opioid pain pills or implanted foreign bodies.
In this type of clinic the methods used are as follow:
1.- Proliferative therapy with hypertonic dextrose. Dextrose is a simple sugar that is made from corn and is chemically identical to glucose or blood sugar. Proliferative therapy is a nonsurgical treatment, which stimulates healing. Short for “proliferation” therapy, Proliferative therapy is also known as nonsurgical ligament and tendon reconstruction injection therapy. Proliferative therapy works by stimulating the body’s own natural healing mechanism to repair injured musculoskeletal tissue. The technique basically reactivates the healing process by injecting a mildly irritating substance such as a somewhat concentrated sugar solution along with the pain killer lidocaine into the injured area to stimulate a temporary low-grade inflammation. This technique has been used since the 1930’s. Proliferative therapy works by causing a temporary, low grade inflammation at the injection site, activating fibroblasts in the area, which, in turn, synthesize precursors to mature collagen and thus reinforce connective tissue.
2.- Plasma rich in plasma . Is a more advanced technique of proliferative therapy that started to be used in the 1990’s. It is a method of injection designed to stimulate healing. “ plasma rich in platelets” is defined as “autologous blood with concentrations of platelets above baseline levels, which contains at least seven growth factors. These again stimulate healing of the pain producing damaged tissues. The blood used to make is your own blood and it is drawn by a venipuncture by your physician. Now even the Mayo Clinic is endorsing this “new” proliferative therapy, for musculoskeletal injuries.
3.- Autologous Bone Marrow Cell concentrate (BMC) injections. This is the most advance orthobiologic medicine technique. However, the term “Stem Cells” has been thrown around by many unscrupulous individual to market thing such as “Amniotic Fluid Stem Cells” or “Umbilical Cord Stem Cells” injections.These products have been proven not to contain any live Stem Cellsby both Dr. Christopher Centeno from Denver Colorado and Lisa Fortier’s lab at Cornell.
The only Cell Procedure of any value in treating musculoskeletal orthopedic conditions is the one in which your own cell are obtained from your bone marrow by a puncture/aspiration with a specialized needle (Jamshidi needle). The aspirate is then concentrated by centrifuge and then injected back into the area needed the same day without adding anything to it. This is the only real Cell procedure for musculoskeletal orthopedic conditions not frowned upon by authorities.
4.- Nano gram dose steroid injections. Yes, we also at times inject steroids but always, always using minimal Nano-gram dose, and always under x-ray or ultrasound guidance to deposit it in the precise place where it will produce the greatest relief while at the same time causing the least damage. Unfortunately we at times face the dilemma of trying to relief the patient discomfort when the patient can not afford the more sophisticated procedures mentioned above because they are not covered by insurance as of yet.
5.- We also use Physical Therapy and at times certain natural supplements that can be obtained at any natural food store, Wall Mart, supermarket or local pharmacy. We do not provide controlled substance prescription!

In some Orthobiologic Medicine/Interventional Orthopedic clinics they make it sound like if this was magic and a cure all. IT IS NOT! In our office we only use it for musculoskeletal conditions such as painful joints, sport injuries and spine conditions. Like with everything else in medicine, there are failures unfortunately.
